Acute services and admissions are based at Royal Surrey County Hospital, Guildford

 

The on-call rota for ENT is shared with middle grades at Basingstoke and Frimley Park Hospitals. 

There are 2 "hot sites" - Frimley and Guildford, each with their own ENT ward and ENT SHO/overnight 1st on-call arrangements. The registrar also covers emergencies from Basingstoke, Frimley, Guildford, Ashford & St Peter's hospitals

Guildford acts as the hub for a network of ENT departments including Ashford and St Peter's hospitals

​

The local area is fairly affluent with many patients being well-read and researched about their symptoms and suspected conditions.

Guildford jobs/rotations

On-call:

  • Weekdays:

    • 8am - 5pm there is no formal on-call registrar

    • On-site registrar is expected to manage emergencies that arise

  • Weeknights: Starting from 5pm until 8am the next day, as non-resident 2nd on-call (1st on-call is an ENT)

    • Covers Basingstoke, Frimley, ​Guildford, Ashford & St Peters' Hospitals

    • There are 2 SHOs on-call (one at Frimley, one at Guildford), 2 consultants on-call (Frimley/Basingstoke and Guildford/St Peter's) and one registrar

  • Weekends: 5pm Friday - 8am Monday. Non-resident on-call registrar as 2nd on-call

  • On-call room? If you live too far to do non-resident on-call at home, on-call room is available:

    • Sign out key from security/reception desk

    • Room in hospital accommodation opposite hospital

Rota/Timetable:

  • BMA Contract compliance:

    • Arrangement for 48hrs off after 8 days of work, or plan to work 7 days max (swaps involved) 

    • Need to book post on-call leave yourself with 6-weeks notice

  • JCST compliance: All job timetables are JCST compliant as stand alone jobs

Bleeping:

Day & night ENT SHO is bleep 0500; 

 

 

ENT ward:

  • Adults admitted to Clandon Ward (Head & Neck ward). outliers often in EAU/SSSU

  • Paediatric overnight stays: Hascombe ward 

  • Treatment room: Clandon ward (out of use currently due to COVID restrictions)

  • Ward round:  8 am daily starting in Jobson room in ENT outpatients

 

SHOs:

  • 8 posts (6 FY2s, 1 GPST2s & 1 core trainee grade)

  • ENT experience?: they will have various experience. ENT training/induction conducted by ENT registrars and consultants on first days of the job

  • Shift patterns:

    • Day on-call: 1 SHO on-call, 1 SHO ward, 1 SHO e-clinic

    • Night on-call  - 1 SHO, No cross-cover

    • ​

Emergency clinic called "eClinic"

  • Booked via: phone call to SHO and e-clinic request form completed by SHO/GP  

  • Runs daily, in outpatients - two per day (morning and afternoon)

  • Run by SHO, who will ask registrar in clinic for advice if needed

  

Theatre:

  • All elective cases should have discharge letters completed at the end of the case

  • Main Theatres or Day Surgery Theatres

  • CEPOD cases in main theatre 

    • How to book​: booking form from theatre front desk & contact coordinator/on-call anaesthetist

  • Pre-op Patients:

    • ​Adults on SSSU/ESU​
    • Paeds on Hascombe

 

Clinics:

  • Outpatient Area 8 at RSCH, Guildford

    • AM clinic start time: 9:00 

    • PM start time : 14:00

  • Ashford & St Peter's: 14:00 Monday afternoons for H&N registrar

  • Additional clinics held at Cranleigh Village Hospital, Haslemere Hospital, (consultants only)

  • Vestibular function tests: Paper form in outpatients

  • BAHA assessments: to be confirmed

  • Sleep studies:

    • Adults:  to be confirmed

    • Paeds: to be confirmed

  • Allergy skin prick tests: to be confirmed

  • Allergy RAST testing: Electronic requests (ICE) If you request, be clear on form what allergens should be tested

  • Outpatient blood tests

  • Local Skull Base Centre: St George's Hospital

  • Local Cochlear implant centre for referrals: Southampton

  • Local Paediatric Tertiary Centre for referrals: St George's 

 

Rotas & leave

  • Updated versions of on-call rota sent out monthly by Debbie Furlong

  • Annual leave - request via Debbie Furlong (debbiefurlong@ nhs.net). She will email them to Mr Walker to approve

  • Study leave - request via Debbie Furlong 

  • Leave request must be submitted with 6-weeks notice

  • Expenses - claim via PGMCE
